No compromise

Clashes of opinion is inevitable when it comes to parenting. Parents always wants the best for their children during those crucial growing up years. I find always doing the best for my son is hard, sometimes.

There’s always interference from outsiders and relatives who are often judgemental on my parenting skills and my opinions regarding children’s diets.

You see…a lot of relatives questioned me on why I don’t encourage my son to take a lot of sweet food and why I throw a fit whenever I saw meddling relatives adding sugars/sweeteners or Milo in to my son’s milk before feeding it to him.

I’ve said it before and I’ve said it again. I want the best for my son and I refuse to tolerate added sugars in his growing up milk. It can be harmful to his body. It not only increases the risk of many health-related issues like obesity, diabetes and dental cavities, but it also triggers hyperactivity and lack of attention in him.

Some people may think that children need a lot of sugar during their growing up years, but I know for sure that it’s not true and just a myth. Too much will do more harm than good to children’s bodies.

Currently, I’m using Anmum Essential for my son. It contains absolutely no added sugars. It’s the only growing up milk powder in the market that contains no added sugars. And because its got the advanced nutrients found in other brands, I don’t feel like I am making a compromise anywhere – it’s the best of both worlds

Parents who have the same problems as me should also take a firm stand. Don’t let meddling in laws, babysitters or relatives undermine your children’s health by allowing them to feed your children excessive added sugars or talk you into buying growing up milks that claim to be superior and specially formulated for growing up children and yet still contain such high levels of added sugars.

I know as parents, we have a lot of woes as people can be judgemental towards our decisions whenever it comes to parenting. But be firm. It’s for our own children’s good. If you’re worried and feel uneasy on what you’re doing and wish to share your experience with other parents like yourself, you can get connected to Mum Knows Best Facebook page.

Mum knows best is a forum for parents such as yourselves. Mummies and daddies alike, you can share your parenting woes, your concerns and parenting tips and tricks with other members. It is real mums talking and sharing with real mums. Its great – do check it out.

I have no doubt that you will benefit from it greatly. You not only can share and exchange your views on parenting and providing the best nutrition for your children during their growing up years, but you also can learn more from each other through it.

No Added Sugar Growing Up Milk

If vehicles needs fuel to move, then the human bodies needs sugar to function properly.  But excessive sugar intake will do more harm to the body than good.

My son cannot take too much sugar in his daily diet. It’s very, very bad for him. I’m sure most of you over here knows why, if you’ve been following the rantings in my blog long enough.

For those who don’t know why it’s bad for the boy… well, I say sugar is not good for my boy not only because it is generally fattening or it causes diabetes, or even high blood pressure or heart attack… no… far from that.

I have to restrict my son from taking too much sugar  because he is a special child. He’s an ADHD (Attention Deficit Hyperactivity Disorder) child with mild Asperger symptoms. For those who does not know about all these medical terms, well, let’s just say that it’s commonly translated as ‘very hyperactive, lack of attention as well as lack of communication and social skills’. Children with such disorders can only be helped via active drug usage, therapy or intervention.

I did not want to use drugs on my boy as I don’t believe in depending on drugs to ‘cure’ the disorder, so I chose therapy and intervention instead. Behavioral therapy and diet intervention is needed in his case.

Apart from diabetes, dental cavities and whatnot, excessive sugar intake is also known to trigger hyperactivity in ADHD children, and therefore, I’m very, very concern about the sugar intake in his diet. I can’t afford my son developing sweet tooth and preference towards sweet food. So whether I like it or not, I have to control his sugar intakes in order to avoid him from being too hyperactive and out of control.

There’s two types of sugar. Natural sugar and added sugar. Both natural and added sugar exists in the form of carbohydrate.

Natural sugar can be found in fruits, veggies and many other class of food in the diet pyramids. Added sugar however are the type of sugar that’s used as additives, preservatives or taste enhancers in various food products, including children’s food.

If you see ‘sucrose’, fructose’ or ‘glucose’ on the labels of the food packaging that you bought, that means you bought food that contains added sugar. And if you see the higher contents of carbohydrate is dictated in any food label, that means the higher the added sugar level is.

Various children’s products such as cereals and growing up milk contains a lot of added sugar, and it’s been quite a headache for me when it comes to choosing the suitable growing up milk for the boy to drink.

Your children may not be ADHD children, but I’m sure most parents are also concern about added sugar in their children’s cereals or milk as excessive sugar intake can cause child obesity, dental cavities, diabetes, etc.

Well now, there’s a solution to that dilemma. At least, when it comes to choosing growing up milk for your children, that is.

Anmum Essential is now available with no added sugar. The newly improved Anmum Essential has absolutely no added sugar. Yes, the new Anmum Essential is a non added sugar product. So far, the new Anmum Essential is the only growing up formula in the market that has no added sugar in it.

Apart from being added sugar-free, also has 100% more DHA than the old Anmum Essential. It’s also fortified with various vitamins, minerals as well as ganglioside. The concentrated formulation also allows you to make more milk with less powder.

Check this out… the feed difference between the old formula and the new formula:

I cannot believe it at first, but the new formula actually lasts longer than the predecessor.

Now, I know children are rather picky when it comes to food and milk. I’m sure most of you are worried about the taste and how your child might respond to the milk, but I assure you, the milk still taste great. I think as parents, we all want the best for our children.

One of the best thing that we can give them is health. To give them health, we need to instill healthy lifestyle and healthy diet in them when they’re still young. We can start by giving them growing up milk that contains no added sugar. No added sugar in your children’s diet means less chances of them going obese, having dental problems or even developing diabetes at a young age.

My son used to take growing up milk that contains added sugar, but when I spotted the New Anmum Essential with no added sugar, I decided to give it a go. There’s no harm trying. I mean, the price is still affordable despite of the price difference. Besides, I know that feeding  my son with milk that contains added sugar will do him more harm than good because of the health issues.

The new formula is around RM10 more expensive than the old formula, but it’s worth it, cuz the new formula is much healthier, and have more servings in each pack.

My son did not exactly have any transition problem when I slowly introduced him to the new Anmum Essential with no added sugar. He didn’t reject the milk but adapted to the taste quite fast as it still taste great, but in case you decided to introduce Anmum Essential with no added sugar as a replacement for your child’s growing up milk and your child happen to have difficulties during the transition period, you can hook up with ANMUM CARELINE  at 1800-88-3122.

Alternatively, if you’re a Facebook user and feels uncomfortable about talking through the phone, you can also discuss your qualms in trying the new Anmum Essential with other parents and Anmum staff in their Anmum Club Facebook page. They have been really helpful in clearing my doubts when I decided to try the new Anmum Essential.

Be sure to spot the difference between the old and the new packaging when you buy Anmum Essential. The new one has ‘no added sugar’/ ‘tanpa gula tambahan’ written on the package.

Vital Signs Monitor

When I was a little girl, I practically lived in the hospital and my friends are usually a bunch of adults who called themselves cardiologist. I had chronic bronchitis as well as a loose valve in my left ventricle, and I had to be closely monitored all the time, especially when I contract a fever for the fear of infection in both my lungs and my heart.

It wasn’t exactly fun to practically grow up in the hospital. They pricked me with needles for blood samples and IV Drip more than they would feed me, and I watched Vital Signs monitors more than I watched the idiot box when I was a little girl.

Do you know what Vital sign monitor is? Well, it’s a machine that functions to measure and display heart rate, breathing rate, and blood pressure on a computer screen. Heart rate, breathing rate and blood pressure are all vital signs in a living human body. If these vital signs become abnormal, an alarm usually sounds. One of the most famous and well know vital signs monitor is Welch allyn vital sign monitor.

These machines are also used during those conscious sedation procedures… just to make sure that nothing goes wrong with the patient. I’ve always hated these machine. It makes me thinks of death. You know… when someone die or goes into a state of comatose, these vital signs machines are the first in line to alarm the medical officers in charge that you’re dead or if there’s something wrong with you because it’ll go ‘BEEP’ very loudly.

When I was forced to overnight in the hospital for monitoring, I always get terrified… terrified of the beeping sounds that will out from those machines, that is. I’ve heard it pretty often in the ward that houses cancer patients. The  beeping sound is really traumatizing. Well, at least to me it is, because it tells me that someone that I might know is gone, and I’ll never see them again.

Anyway, it is a good thing that I was a sickly child and faced near death experience more than anyone else. It made me appreciate life more, and it also helps me understand a lot of illness and be more emphatic towards patients and their families.

Cleffairy: If someone tells me if they’re sick and was thrown into the operation room, I can tell if they’re telling the truth or fibbing by just listening to their description about the procedures that has been performed on them. So… if you happen to be my friend or my student, don’t dream of bluffing me if you’re not sick, because I will know if you lie!

Health food: Whey Protein

I have a bestie, or rather, a partner in crime who is now working as a personal trainer in a local gym. We grew up together but we’re like heaven and Earth. We’re totally different beings, but I think we complement and get along with each other just fine. After all we have been friends since we were 11 years old.

My bestie, she’s a very health conscious person. Which means, she goes to gym to work out even before she started to venture into the fitness and body building industry. While me, on the other hand, is somewhat nonchalant when it comes to health and fitness. She’s the kind who would count her calorie intakes as she eat while I don’t really care about calories. As long as the food taste good, anything goes, even if it’s unhealthy.

She could go on and on and on, talking about health supplements and why whey protein is essential in body building and fitness industry. Truthfully, I am not a bit familiar about health supplements as well as whey proteins and whatnot. I am practically illiterate when it comes to these sort of things.

And so, I looked up supplements UK where there are loads of infos on health food supplements provided.

According to supplements UK, whey protein is is a high quality protein powder made from cow’s milk. Milk has two proteins: Casein (approximately 80%) and Whey Protein (approximately 20%).

Whey protein is more soluble than casein and also has a higher quality rating. It is often referred to as the gold standard of protein as it is the most nutritious protein available.

Whey protein is a high quality, complete protein, with all the essential amino acids and is also the richest known source of naturally occurring branched chain amino acids (leucine, isoleucine and valine). And so, whey protein are important for active individuals, individuals who exercise a lot, bodybuilders and professional athletes.

So is whey protein for me? No. Not quite. It’s not quite for me, cuz I I don’t exercise much. I only swim a few times a month, and that’s merely for relaxing, not exercising. But whey proteins is certainly for sizzling hot and virile men like this. 😀

And this…

And definitely this…

Good grief, working out and taking whey protein does that to one’s body? Hmmm… I better stop and run to the nearest gym for some eye candies instead before my imaginations runs wilder. 😀 Anyone with me?

Cleffairy:  I kinda know why my bestie got so hooked in the gym. Tsk tsk tsk! Shame on you, Pau! 😛

AIDS is a mass murderer

I haven’t been really paying attention on writing my blog these days. Something else were occupying my mind and body. I was busy writing my novel, and still writing though NaNoWriMo 2009 is over. I won. And I wanted so much to gloat about my achievements, but that will have to wait another day, because today, 1 Dec 2009, is World AIDS Day.

And because today is World AIDS Day, I’ve ‘invited’  VIPs to give you all a message. They have something to tell you. Let’s see if you readers can recognize who they are, and what these people have in common with each other.

Here’s what they have to say:

These people knows what they are talking about. They killed PEOPLE, after all. AIDS is no difference. AIDS kill people too.

Consummation of love instead of casual sex makes a difference. Being faithful makes a difference. Being monogamous makes a difference.

Playing it safe makes a difference

Stop victimizing the innocents. They are our mothers, our sisters, our daughters.

They are our brothers…

They are our children, our future

They are you, me and us

AIDS does not discriminate. So…

Why I Love My Cat

Image062

Usually, I don’t do random entries that will sound pathetic and childish like this. But I am very much in need for a writing therapy at the moment, because I am not happy and definitely feeling down. I’ve been through a lot this year, and I can definitely say that this is one of my worst year ever.

Too many things happened, and I sometimes wished for that there’s more than 24 hours a day. Time is simply not enough, and I tend to wonder if I am having a post traumatic stress, seeing how often I feel so down and depressed since the fire happened.

Long long time ago, I never like any animals to keep as pet. Most of them stinks and smell like poo all the time, and not to mention if it’s a feline or a canine, they need treatments and grooming pretty often too.

I adopted a stray kitten 3 month ago. I did not even know why I brought it home. I really did not know why. It’s not even cute to even begin with. The kitty, whom I called Meow Meow looks like a New Zealand dairy cow with the patchy black fur. But I suppose it is fate that Meow Meow comes to live with me for me and my entire family owe her our lives. To cut the story short, she saved us from the fire that burned down my house on 18th July 2008- four days after I brought her home. I suppose, she’s pretty much godsent.

I was no animal lover, but I came to realize that pets could bring so much joy and serenity to the soul. I used to think that cats are stupid creature that vomits furball everywhere and poop around, but boy was I wrong. My kitty learned to use the toilet all by herself, and all I need to do was just flush the toilet each time she poop or pee. She’s pretty clever for a not so cute kitten.

There’s a few things that I noticed about Meow Meow. Among them is she loves to watch superheroes shows and old shows like Knight Rider and Air Wolf. She’s pretty choosy when it comes to food, and prefers to eat wet food more than the nasty dry pet food for cats. If I did not bathe her at least once a week, she’ll get very uncomfortable and bites and scratch around angrily.

Meow Meow also loves to be petted and hugged, and when it’s raining or storming outside, she’ll stick to me or my husband like a glue. I used to think that kittens only loves to chase yarn balls, but Meow Meow is different. She love all sort of children’s toys and loves to see picture books and not to mention watch cartoon. Sometimes, she even behaved like a little girl. She’ll want blanket and tends to hug little soft toys when she sleeps.

These days, Meow Meow would give me a wake up call. She would lick my face in the morning and if I still refused to get my lazy butt up from the bed, she’ll claw me. I never knew that I could love a little kitten so much that sometimes I treat her like my own daughter.

I could list down many reasons why I love Meow Meow. But at the end of the day, having someone to tell my problems to and would keep my secrets safe tops the list. For what it’s worth, I am sure glad and thankful that I brought Meow Meow home.

Cleffairy: Sometimes, animals would understand you better than human. Well, at least they won’t backstab you, scold you or even judge you for what you did and did not do. At least, pet’s love and devotion for you is always genuine.

S.T.R.E.S.S and D.E.P.R.E.S.S.I.O.N is a silent killer

There’s been so many bad news around me lately. There have been sudden and unexpected loss, and many more tragic events had unfortunately occurred. Though misfortune does not directly befalls me, but it does befalls to the people whom I am fond of, and seeing that they are upset and depressed deeply saddens me. It breaks my heart to see the ones that I love fall into stress and pulled into depression.

Those who are in my blogging circle for some time now would know what I am talking about. But I do not wish to mention clearly what I am really talking about. I respect people’s privacy and I’ll be damned if I gossip around like some sexually deprived housewives about people’s misfortune. That would be a tragedy more than the event itself.

Sometimes I do hope that they are stress free, but then again, things happens. Things are sometimes beyond our control, and it’s already predestine. I could easily advice the people who are down, moody, stressful and depressed to not think too much of their problem, but I too had experienced terrible stress and been pulled into a horrible depression that almost made me suicidal once. It takes me more than 2 years to get out of one. And I am not sure if I’m safely rid of it. So I know that brushing off people’s stress and take the easy way out by telling them not to think to much and try to chill is a definite no no!

People who are stressed up needed help, not such brush off, and most are desperate enough that they are almost suicidal. Do you honestly think that telling people not to think about their problems would work? People wanted solution to their problems. People wanted to get out of their problems. Telling them to chill and relax or just pray to God without making any efforts would only heighten their depression, because, such things do not provide any solution. God help those who helped themselves. Who dare to tell me that God had personally helped them? Tell me so, I dare you! I stopped believing in God because when I was almost suicidal, it was not God who helped me through it.

I know this well because I once experienced it myself. What experience, you ask me? Why, depression of course. As I stated before, I was almost suicidal because of it. Life did not treat me well. Relationship was bland and awry. Support from family members/spouse was not with me, because they taught I had no such issue or perhaps ignorant of it.

Maybe that, or they are not sensitive and turn blind eyes to it. Loneliness and lack of social communication/interaction made me feel trapped in my own environment and felt completely left out. I felt that there was no purpose in life. And of course I seek ways to escape it, and I suppose, suicide was the best form of escapism. Yes, I truly thought it was. I was ready to leave, and would gladly do so without second thought. I was happy when I thought that my death would finally free me from all sad things and disappointments that I experienced. I will finally be rid of my nasty unresolved problems.

I was happy with the thoughts that the ones I left behind would suffer and regret that I finally died and it’s their fault that I decided that suicide is way better than be with them. Serve them right for not being attentive. Serve them right for being such uncaring, insensitive bastards who abandoned me without sparing a single thoughts for me. Serve them right for making me feel left out and made me feel that I am not needed, unloved and have no purpose in life.

When I suffered depression, the thought of death was nothing to me. It was imprinted in my thoughts that Death comes to everyone anyway. I was tempted to die. The thoughts was pleasant and inviting. It does not make any difference to me on how it’s delivered and when it is. People die everyday, and so, what difference would it make to me anyway? I seriously thought that.

People who are depressed and melancholic WILL NOT try to chill or not think too much. It doesn’t work that way. Personally, when I am depressed, I would like someone to LISTEN to me instead of judging my situation. People who are depressed need a good pair of ears and a pair or mouth that will shut tight and sealed our problems inside and not broadcast it to the world. But unfortunately, good ears and silent mouth are extremely hard to come by. The things that I bottled up inside are the kind of things that i would not even share with my best friend. Those kind of mouth and ears are hard to come by, unless of course, you pay for it by seeking professional help.

I would shamelessly admit that I’m mentally disturbed enough and need a trustworthy therapist. I wanted help. I want a safe way out and I do not wish to feel inferior anymore. I do not wish to be one of those who gives up on life because they have issues that they thought could not be solved. But help are expensive and hard to come by in Malaysia. And then comes the stigma that everyone who seeks psychologist are lunatic. I could not afford to do so, because financial restriction is also a tiny part of the issue that drives me into the pit of depression.

Stress and depression is a silent killer. It brings many health problems. It hurts. Very much. At least to me it does. Blood pressure skyrocketed, heart pumps harder and feels painful, and stomach rebelled by not digesting and consequently caused horrendous stomachache and vomiting. Health then will deteriorate terribly. You’ll look sickly and unhappiness will be written clearly on your facial expression.

When you are depressed, it’s not necessarily take suicide to finally end your misery. But terrible state of health would deliver you to the underworld itself in the form of stroke, high blood pressure of even heart attacks. I envy my Western counterparts. Psychologist are cheaper per session and easier to come by, and there’s no such stigma because Westerners are courageous enough to admit that they are human enough to need help and it’s necessary for them to seek professional help.

I was in serious depression back in 2006, 2007 and 2008. Serious as in really serious. Almost suicidal. My life was about to be snipped away back then. I could not get a grip, and I mope about all the time. I lost control of my own life because of the lack of social communication and people who understood me. I feel trapped and lost my purpose in life and it was almost my undoing. I felt that I’m being placed in a small corner by people whom I love and as a result, I shut them out of my life by not disclosing my problems from them and bottles everything inside. I felt that it’s useless telling anyone anyway. They would have not understood and there’s no point in making such effort because I’m convinced that it will not provide me any solution. Things will continue to be the same, so, what’s the point wasting my breath when there is no desirable outcome to my outpouring of emotions?

I am not sure if i finally did recover from depression, because I still feel easily depressed every now and then. And pretty easily too. But I found that DISTRACTION can be a good diversion to stress and depression. Distraction doesn’t have to always be a healthy one, but it can save life. And, as they say, misery loves company. Find someone you trust to share your problems with. That may not do much, but at least some part of it will be lift up from you. And it does help if you could do something about your low self-esteem.

Perhaps I should write another article following this one as a follow up on how one could handle stress. Until next time people, this is getting way too long. My sincerest apology if I bore you out of your skull. And I’d thank you if you share your own experience on stress and depression if you have any.

Cleffairy: Truthfully deep inside, I am a battered person. Sometimes, I think I managed to still be alive because I’m anger motivated and I turn vengeful. I wanted revenge on those who made me miserable.

Infos on H1N1

This won’t be a pretty entry, but then again, I think it might be useful for those who are curious about the swine flu/ H1N1 that is now on phase 5 as announced by WHO. Do remember that prevention is always better than cure. Informations are courtesy of WHO, as of 1st May 2009.

How do people become infected with influenza A(H1N1)?

Outbreaks in humans are now occurring from human-to-human transmission. When infected people cough or sneeze, infected droplets get on their hands, drop onto surfaces, or are dispersed into the air. Another person can breathe in contaminated air, or touch infected hands or surfaces, and be exposed. To prevent spread, people should cover their mouth and nose with a tissue when coughing, and wash their hands regularly.

What are the signs and symptoms of infection?

Early signs of influenza A(H1N1) are flu-like, including fever, cough, headache, muscle and joint pain, sore throat and runny nose, and sometimes vomiting or diarrhoea.

Regarding study of the first outbreak, have you received any feedback from the WHO team sent to Mexico to investigate the outbreak?

Teams are already sending epidemiological evidence but we will know more over the next few days.

Is there any confirmation of transmission between pigs and humans at this point?

No.

Is there any information on the economic impact of the outbreak so far?

No.

Why are we so worried about this pandemic possibility when thousands die every year from seasonal epidemics?

Seasonal epidemics occur every year and we are able to treat the virus with seasonal vaccines. A pandemic is a worldwide epidemic. It is a new virus and one to which the populations will have no immunity.

What does pandemic alert phase 5 mean?

According to WHO definitions of phases – the virus has caused sustained community level outbreaks in at least two countries in one WHO region. At phase 5 a pandemic is considered imminent.

What is sustained human to human transmission?

This occurs when three generations are involved – i.e. the virus has passed from person A to person B and then to person C.

What is the difference between a probable case and a confirmed case?

A probable case is one that shows the symptoms of influenza such as cough, fever, headache, chills and fatigue and either tests positive for type A influenza or is considered linked to another probable or confirmed case.

A confirmed case is one which tests positive in a laboratory for the influenza A(H1N1) virus.

At what point does WHO consider a pandemic to have started?

Phase 6, as defined by the WHO pandemic preparedness guidelines. However, during both phase 5 and phase 6, national and local actions to respond to the outbreak shift from preparedness to response at a global level. The goal of recommended actions during these phases is to reduce the impact of the pandemic on society.

Is an effective vaccine already available against the new influenza A(H1N1) virus?

No, but work is already under way to develop such a vaccine. Influenza vaccines generally contain a dead or weakened form of a circulating virus. The vaccine prepares the body’s immune system to defend against a true infection. For the vaccine to protect as well as possible, the virus in it should match the circulating “wild-type” virus relatively closely. Since this H1N1 virus is new, there is no vaccine currently available made with this particular virus. Making a completely new influenza vaccine can take five to six months.

What implications does the declaration of a pandemic have on influenza vaccine production?

Declaration by WHO of phase 6 of pandemic alert does not by itself automatically translate into a request for vaccine manufacturers to immediately stop production of seasonal influenza vaccine and to start production of a pandemic vaccine. Since seasonal influenza can also cause severe disease, WHO will take several important considerations such as the epidemiology and the severity of the disease when deciding when to formally make recommendations on this matter. In the meantime, WHO will continue to interact very closely with regulatory and other agencies and influenza vaccine manufacturers.

How important will influenza A(H1N1) vaccines be for reducing pandemic disease?

Vaccines are one of the most valuable ways to protect people during influenza epidemics and pandemics. Other measures include anti-viral drugs, social distancing and personal hygiene.

Will currently available seasonal vaccine confer protection against influenza A(H1N1)?

The best scientific evidence available today is incomplete but suggests that seasonal vaccines will confer little or no protection against influenza A(H1N1).

What is WHO doing to facilitate production of influenza A(H1N1) vaccines?

As soon as the first human cases of new influenza A(H1N1) infection became known to WHO, the WHO Collaborating Center in Atlanta (The Centers for Disease Control and Prevention (CDC) in the United States of America) took immediate action and began the work to develop candidate vaccine viruses. WHO also initiated consultations with vaccine manufacturers worldwide to facilitate the availability of all necessary material to start production of influenza A(H1N1) vaccine. In parallel, WHO is working with national regulatory authorities to ensure that the new influenza A(H1N1) vaccine will meet all safety criteria and be made available as soon as possible.

Why is WHO not asking vaccine manufacturers to switch production from seasonal vaccine to a influenza A(H1N1) vaccine yet?

WHO has not recommended stopping production of seasonal influenza vaccine because this seasonal influenza causes 3 million to 5 million cases of severe illness each year, and kills from 250 000 to 500 000 people. Continued immunization against seasonal influenza is therefore important. Moreover, stopping seasonal vaccine production immediately would not allow a pandemic vaccine to be made quicker. At this time, WHO is liaising closely with vaccine manufacturers so large-scale vaccine production can start as soon as indicated.

Is it possible that manufacturers produce both seasonal and pandemic vaccines at the same time?

There are several potential options which must be considered based on all available evidence.

What is the process for developing a pandemic vaccine? Has a vaccine strain been identified, and if so by whom?

A vaccine for the Influenza A(H1N1) virus will be produced using licensed influenza vaccine processes in which the vaccine viruses are grown either in eggs or cells. Candidate vaccine strains have been identified and prepared by the WHO Collaborating Center in Atlanta (The Centers for Disease Control and Prevention (CDC) in the United States of America)1. These strains have now been received by the other WHO Collaborating Centers which have also started preparation of vaccine candidate viruses. Once developed, these strains will be distributed to all interested manufacturers on request. Availability is anticipated by mid-May.

How quickly will influenza A(H1N1) vaccines be available?

The first doses of Influenza A(H1N1) vaccine could be available in five to six months from identification of the pandemic strain. The regulatory approval will be conducted in parallel with the manufacturing process. Regulatory authorities have put into place expedited processes that do not compromise on the quality and safety of the vaccine. Delays in production could result from poor growth of the virus strain used to make the vaccine.

How would manufacturers be selected?

There are currently more than a dozen vaccine manufacturers with licenses to produce influenza vaccines. Upon request, the vaccine strain will be available to each of them, as well as to other qualified vaccine manufacturers who are preparing to make influenza vaccine but do not yet have a licensed influenza vaccine.

What is the global manufacturing capacity for a potential influenza A(H1N1) pandemic vaccine? Is this the same as the global manufacturing capacity for H5N1?

The projections made for the production capacity of an vaccine for H5N1 cannot be automatically assumed to be the capacity to make an H1N1 vaccine. H5N1 and H1N1 viruses are different and the amount of antigen needed to make an effective H1N1 vaccines may be different than for H5N1. Therefore it is not possible to make a precise estimate. However, given these considerations, a conservative estimate of global capacity is at least 1 to 2 billion doses per year.

How is production capacity for influenza vaccines distributed geographically?

More that 90% of the global capacity today is located in Europe and in North America. However, during the past five years, other regions have begun to acquire the technology to produce influenza vaccines. Six manufacturers in developing countries have done so with technical and financial support from WHO.

What will be the storage requirements for influenza A(H1N1) vaccine?

The vaccine should be stored under refrigerated conditions at between 2°C and 8°C.

It has been impossible so far to develop vaccines for major killers such as HIV and malaria. How sure are we that there will not be scientific or other hurdles in developing an effective influenza A(H1N1) vaccine?

Typically, development of influenza vaccines has not posed a problem. Influenza vaccines have been used in humans for many years and are known to be immunogenic and effective. Each year seasonal influenza vaccines with varying composition are produced for the northern and southern hemisphere influenza seasons. Vaccine manufacturers will employ a number of different technologies to develop their vaccines. They will take advantage, notably, of novel approaches that were developed over the past years for H5N1 avian influenza vaccines. One key unknown is yield of vaccine virus production, since some strains grow better than others and the behavior of the new influenza A(H1N1) strain in manufacturers’ systems is not yet known. New recombinant technologies are under development, but have not yet been approved for use.

Will influenza A(H1N1) vaccines be effective in all population groups?

There are not data on this but there also is no reason to expect that they would not, given current information.

Will the influenza A(H1N1) vaccine be safe?

Licensed vaccines are held to a very high standard of safety. All possible precautions will be taken to ensure safety of new influenza A(H1N1) vaccines.

How can a repeat of the 1976 swine flu vaccine complications (Guillain-Barré syndrome) experienced in the United States of America be avoided?

Guillain-Barré syndrome is an acute disorder of the nervous system. It is observed following a variety of infections, including influenza. Studies suggest that regular seasonal influenza vaccines could be associated with an increased risk of Guillain-Barré syndrome on the order of one to two cases per million vaccinated persons. During the 1976 influenza vaccination campaign, this risk increased to around 10 cases per million vaccinated persons which led to the withdrawal of the vaccine.

Pandemic vaccines will be manufactured according to established standards. However, they are new products so there is an inherent risk that they will cause slightly differently reactions in humans. Close monitoring and investigation of all serious adverse events following administration of vaccine is essential. The systems for monitoring safety are an integral part of the strategies for the implementation of the new pandemic influenza vaccines. Quality control for the production of influenza vaccines has improved substantially since the 1970s.

Will it be possible to deliver new influenza A(H1N1) vaccine simultaneously with other vaccines?

Inactivated influenza vaccine can be given at the same time as other injectable vaccines, but the vaccines should be administered at different injection sites.

If the virus causes a mild pandemic in the warmer months and changes into something much more severe in, say, 6 months, will vaccines being developed now be effective?

It is too early to be able to predict changes in the influenza A(H1N1) virus as it continues to circulate in humans or how similar a mutated virus might be to the current virus. Careful surveillance for changes in the influenza A(H1N1) virus is ongoing. This close and constant monitoring will support a quick response should important changes in the virus be detected.

Will there be enough influenza A(H1N1) vaccine for everyone?

The estimated time to make enough vaccine to vaccinate the world’s population against pandemic influenza will not be known until vaccine manufacturers will have been able to determine how much active ingredient (antigen) is needed to make one dose of effective influenza A(H1N1) vaccine.

In the past two years, influenza vaccine production capacity has increased sharply due to expansion of production facilities as well as advances in research, including the discovery and use of adjuvants. Adjuvants are substances added to a vaccine to make it more effective, thus conserving the active ingredient (antigen).

What is WHO’s perspective on fairness and equity for vaccine availability?

The WHO Director-General has called for international solidarity in the response to the current situation. WHO regards the goal of ensuring fair and equitable access by all countries to response measures to be among the highest priorities. WHO is working very closely with partners including the vaccine manufacturing industry on this.

Who is likely to receive priority for vaccination with a future pandemic vaccine?

This decision is made by national authorities. As guidance, WHO will be tracking the evolution of the pandemic in real-time and making its findings public. As information becomes available, it may be possible to better define high-risk groups and to target vaccination for those groups, thus ensuring that limited supplies are used to greatest effect.

Will WHO be conducting mass influenza A(H1N1) vaccination campaigns?

No. National authorities will implement vaccination campaigns according to their national pandemic preparedness plans. WHO is exploring whether the vaccine can be packaged, for example, in multi-dose vials, to facilitate the rapid and efficient vaccination of large numbers of people.

Developing countries are very experienced in administering population-wide vaccination campaigns during public health emergencies caused by infectious diseases, including diseases like epidemic meningitis and yellow fever, as well as for polio eradication and measles control programmes.

How feasible will it be to immunize large numbers of people in developing countries against a pandemic virus?

Developing countries have considerable strategic and practical experience in delivering vaccines in mass campaigns. The main issue is not feasibility, but how to ensure timely access to adequate quantities of vaccine.

What is the estimated global number of doses of seasonal vaccine used annually?

The current annual demand is for less than 500 million doses per year.

Will seasonal influenza vaccine continue to be available?

At this time there is no recommendation to stop production of seasonal influenza vaccine.

Is it safe to travel?

WHO is not recommending travel restrictions related to the outbreak of the influenza A(H1N1) virus. Today, international travel moves rapidly, with large numbers of individuals visiting various parts the world. Limiting travel and imposing travel restrictions would have very little effect on stopping the virus from spreading, but would be highly disruptive to the global community.

Influenza A(H1N1) has already been confirmed in many parts of the world. The focus now is on minimizing the impact of the virus through the rapid identification of cases and providing patients with appropriate medical care, rather than on stopping its spread internationally. Furthermore, although identifying the signs and symptoms of influenza in travellers can be an effective monitoring technique, it is not effective in reducing the spread of influenza as the virus can be transmitted from person to person before the onset of symptoms. Scientific research based on mathematical modelling indicates that restricting travel will be of limited or no benefit in stopping the spread of disease.

Historical records of previous influenza pandemics, as well as experience with SARS, have validated this point.

Travellers can protect themselves and others by following simple recommendations related to travel aimed at preventing the spread of infection. Individuals who are ill should delay travel plans and returning travellers who fall ill should seek appropriate medical care. These recommendations are prudent measures which can limit the spread of many communicable diseases and not only Influenza A(H1N1).

s it safe to eat pork and pork products?

Yes. influenza A(H1N1) has not been shown to be transmissible to people through eating properly handled and prepared pork (pig meat) or other products derived from pigs. The influenza A(H1N1) virus is killed by cooking temperatures of 160°F/70°C, corresponding to the general guidance for the preparation of pork and other meat.

What are the recommendations for face masks?

If you are not sick you do not have to wear a mask. If you are caring for a sick person you should wear a mask. All home made masks should be cleansed regularly.

If you are sick, stay at home and avoid contact with people.

How can I protect myself and prevent illness?

Practise general preventive measures for influenza to prevent infection:

  • avoid close contact with people who appear unwell and have fever and cough;
  • wash your hands with soap and water thoroughly and often;
  • practise good health habits including adequate sleep, eating nutritious food, and keeping physically active.

How do I care for an ill person at home?

  • Separate the ill person from others, keeping the person at least 1 metre in distance from others.
  • Cover your mouth and nose when caring for the ill person. Either commercial or homemade materials are fine, as long as they are disposed of or cleaned properly after use.
  • Wash your hands with soap and water thoroughly after each contact with the ill person.
  • Improve the air flow where the ill person stays. Use doors and windows to take advantage of breezes.
  • Keep the environment clean with readily available household cleaning agents.

If you are living in a country where there are infections follow additional advice from your national and local health authorities.

What should I do if I think I have the illness?

If you feel unwell, have high fever, cough or sore throat:

  • Stay at home and keep away from work, school or crowds.
  • Rest and take plenty of fluids.
  • Cover your mouth and nose with disposable tissues when coughing and sneezing, and dispose of the used tissues properly.
  • Wash your hands with soap and water often and thoroughly, especially after coughing or sneezing.
  • Inform family and friends about your illness and try to avoid contact with other people.

What should I do if I need medical attention?

  • Contact your doctor or healthcare provider before travelling to a health facility, and report your symptoms. Explain why you think you have influenza A(H1N1) (if you have recently travelled to a country where there is an outbreak in humans). Follow the advice given to you.
  • If it is not possible to contact your healthcare provider in advance, communicate your suspicion of infection as soon as you arrive at the facility.
  • Cover your nose and mouth during travel.

Are some people more at risk?

More study is needed to determine if some populations (i.e. younger or older people, or people with other medical conditions) could be affected by the outbreak, of if they are at higher risk for severe illness. WHO recommends that everyone take precautions to prevent the spread of infection.

Are there any special recommendations for pregnant women?

Yes, they are vulnerable. Like everyone, they should take all the necessary precautions.
